What the web design questionnaire asks
Your client sees one question at a time and answers with the keyboard or a tap. Here is question 7, where the sitemap starts.

Which pages does the new site need?
Select all that apply. The home page is a given.
Multiple Choice with no cap on selections, so the answer doubles as a first sitemap. Every screen carries your logo, colors and font. See how conversational forms behave.

How to get useful answers from a website questionnaire
A website questionnaire is the first draft of the project, written by the client. Done well, it hands you a sitemap, a feature list, the state of the content and a budget before anyone books a call. Done badly, it is a forty-question PDF that comes back half empty two weeks later, and you ask everything again at the kickoff.
Ask what the client can answer, keep the rest for the call
Clients know their business, their customers and their deadline. Few know whether they need a headless CMS, and asking makes them guess. Every question here is one a business owner can answer from memory. Hosting, the CMS and integrations are yours to raise on the call, once you know what the site has to do. That is why the template stops at 16 questions instead of 40.
Lead with the job, not the look
Question 5 asks what the site is for before anything about pages or style: win new leads, sell products online, show our work, or help existing customers. When the job and the pages disagree (a lead generation site with no contact page), you have your first talking point. Question 6 asks who the site is for, and its help text asks for one real customer, not a demographic: “a practice manager at a three-dentist clinic” beats “B2B, 30 to 55”.
Turn pages and features into a sitemap
Questions 7 and 8 are Multiple Choice, so ticking boxes costs the client nothing and you get a rough sitemap and a scope list in one pass. The features list covers a contact or quote form, an online store, booking requests, a newsletter signup, site search, more than one language and a member login. Keep “Something else” on both lists, and cap the features question at three selections if you want priorities rather than a wishlist.
Find out where the content stands
Content is what usually makes a web project late, and clients rarely volunteer that nothing is written. Question 11 asks plainly: written and approved, some of it exists, nothing yet, or we would like help writing it. “Nothing yet” next to a launch six weeks out is a conversation to have before you sign, and a chance to quote for copywriting.
Brand files and reference sites
The file upload question takes one PDF, PNG or JPG of up to 10 MB, enough for a brand guide or a logo export. Vector logos and fonts can follow through your usual file sharing. Question 13 asks for one site the client admires, and question 14 appears only after they paste a link, asking what they like about it. A link alone tells you little; “the way the case studies open full screen” tells you a lot.
Why budget is a slider
Plenty of clients who would never type a number will drag a handle. The template ships with a range of 2 to 60, in thousands of dollars. Move the minimum up to your real floor. The handle rests at the middle of the range but records nothing until the client moves it, and a required slider cannot be skipped. That middle is also an anchor, so pick a range whose midpoint you would be glad to hear. Prefer price bands? Swap it for Single Choice, as the client intake form template does.
Read the launch date as urgency
Most people answer the date approximately, and that still separates “before the trade show in March” from “sometime this year”. When it is close, ask what is driving it: a fixed event turns “can we add a member area?” into “yes, in phase two”.

Store questions only appear for stores
Two rules ship switched on, in the same Logic panel every form uses: AND/OR conditions, 20 operators and four actions (show, hide, jump and end form). The first keeps e-commerce questions away from anyone not building a shop.

Rule Summary
When Main job of the site is any of "Sell products online" OR Features is any of "Online store" → Show field: How many products will you sell at launch?, Where do you sell today?
Rule 1, opened. The conditions are joined with OR, so either answer opens the store questions.
9 →
How many products will you sell at launch?
A rough count is fine. It shapes the catalog and the build time.
Only people building a shop see this screen. Question 10 asks where they sell today, so you know whether you are migrating a catalog or starting one.
Rule 2 shows “What do you like about it?” only when question 13 is answered. Want a budget floor too? Add a rule that ends the form when the slider is less than your minimum. Every operator is on the conditional logic page.

Where the questionnaires land
Every questionnaire lands in that form's responses table, with search, a date filter, CSV export and a drawer holding every answer and the brand file. Then send it on to wherever you run projects.
Each one connects with your own account, and names, emails and companies also roll up into Contacts. Browse every destination on the integrations page, or see how studios run the whole intake with client onboarding forms.

Is this website questionnaire template free?
Yes. Cloning the template, publishing it and collecting answers all sit on the Upform free plan, which caps neither forms nor responses and includes 100 MB of file storage. Your published form shows a small Upform badge; Pro is $12 a month and removes it, lifts the storage limit and adds up to five team members.
What should a web design questionnaire include?
Contact details, the main job of the site, the audience, the pages and features needed, how ready the content is, brand files, a reference site, the budget and the launch date. Those answers give you a first sitemap, a feature list and a feasibility check before the kickoff call. Leave the CMS and hosting for the call itself.
How do the e-commerce questions work?
A logic rule shows questions 9 and 10 only when someone picks Sell products online as the main job or ticks Online store as a feature. Everyone else skips straight to the content question. Edit or delete the rule in the Logic panel.
Can clients upload their logo and brand guide?
Yes. Question 12 is an optional File Upload that takes one PDF, PNG or JPG of up to 10 MB. The file is stored with that form's responses and opens from the response drawer. Add a second upload question if you also want photos.
Why is the budget question a slider?
Plenty of clients who will not type a number will still drag a handle, so a slider often gets an answer where a text box stays blank. Start the range at your real minimum. The handle records nothing until it is moved, and you can swap it for Single Choice price bands in a minute.
Can the questionnaire carry my studio's branding?
Yes. Add your logo, colors, a Google font and a background image in the Design panel, or start from one of four theme presets. Publish it on a link, embed it with an iframe or paste the email button into a message. The free plan shows a small Upform badge; Pro removes it.
