Pick the right form

Employment application form or job application form?

Both are free Upform templates. This one is the general form for volume and shift hiring. The job application form template is shorter and leans on the CV, for salaried roles.

Before you publish

What to include on an employment application form

An employment application puts every applicant's answers in the same order, so a store manager can compare forty people for a weekend shift in one table. That only works if the form asks for what you will use and nothing else.

Personal details: contact, not identity

Name, email and phone number are enough to reach someone. For location, question 4 uses the Address type with only city, state and ZIP switched on: enough to judge the commute, without a street address you will not need until payroll.

Put availability at the center

For shift work, when someone can work matters more than where they worked. Questions 7 and 8 are Multiple Choice, so applicants pick every day and shift, and question 9 takes the earliest start date. With the kind of work they want (full-time, part-time or seasonal), that is enough to build a shortlist from a CSV export.

Eligibility is a yes or no

Ask whether someone is legally eligible to work in the country, not their citizenship or nationality. Document checks come later: in the US, the Form I-9 is completed after a job offer is accepted.

Keep history, education and references short

A dozen fields per past job is too much to type on a phone. Question 12 asks for the last three jobs in one Long Text answer, with a help line listing employer, job title, dates and reason for leaving. Education is one choice of the highest level completed; delete it if the role has no requirement. References are a second Long Text asking for two names and a phone number or email, with a note that you only call them after an interview.

Make the resume optional

Plenty of good hourly applicants do not keep a resume, and fewer have one on their phone. The template's file upload question is optional and accepts PDF, PNG and JPG up to 10 MB, so a phone photo of a printed resume works too. The free plan's 100 MB holds a few hundred PDF resumes, fewer if most arrive as photos.

Then check the numbers After the first twenty applications, open Results. Starts, submissions and completion rate show whether people are finishing. If completion is low, shorten the history and references questions first. More in our guide to form abandonment.

Lawful and lean

What to leave off an employment application

Asking creates risk even if you never read the answer, and every sensitive detail you collect is one more thing to store, protect and delete on request.

Leave off the application

  • Date of birth or age
  • Social Security number, passport or other government ID
  • Marital status, children or pregnancy
  • A photo of the applicant
  • Health, disability or medical history
  • Religion, national origin or citizenship status
  • Salary or criminal history, where local law bans asking

Ask this instead

  • Are you at least 18? A Yes/No, and only where the role has a legal minimum age
  • Are you legally eligible to work in this country?
  • Can you perform the essential duties of this role, with or without reasonable accommodation?
  • Do you hold a valid driver's license? Only for roles that involve driving
  • ID numbers, bank details and tax forms collected at hire, through payroll

General guidance, not legal advice: rules differ by country, state and city. For the data side, see our notes on GDPR-compliant forms.

Question 16

A declaration instead of a signature

Paper employment applications usually end with a signature line. Upform has no signature field, so the template closes with a required Yes/No question asking the applicant to confirm their answers are true and complete.

  • Required. A No still submits, so check the answer before you call anyone.
  • Need a legally binding signature, for example on an offer letter? Use your e-signature tool at that stage.
  • Running background checks in the US? The FCRA disclosure must be a standalone document, so keep it out of this form.

The logic that ships with the template

Two rules ship switched on. Each is a set of AND/OR conditions with one of four actions: show, hide, jump or end form.

Rule 1: end early if someone is not eligible to work

If question 10 comes back as No, the form saves the answers so far and ends, instead of asking for a work history you cannot use. Sponsor visas or hire abroad? Switch the rule off.

If “Eligible to work” is exactly NoThen end form

Rule 2: only driving roles see the license question

Question 11 stays hidden unless the position picked at question 5 is Delivery driver. Everyone else goes straight from eligibility to work history.

If “Position” is exactly Delivery driverThen show “Driver's license”

Change either rule or add your own; every operator is listed on the conditional logic page.

Every application lands in your responses table first. Then send it onward.

Each one connects with your own account. To take applications on your careers page instead of a link, embed the form with one iframe snippet. Every destination is on the integrations page.

Frequently asked questions

Is this employment application form template free?

Yes, and it stays free. Cloning, publishing and collecting applications all run on the Upform free plan: unlimited forms and responses, all 21 question types, logic and every integration, with 100 MB of file storage and a small Upform mark. Pro is $12 a month and removes the mark, lifts the storage limit and adds up to five team members.

What is the difference between an employment application form and a job application form?

An employment application is the general form for hourly, shift and seasonal hiring. It asks every applicant for availability, eligibility, work history, education and references in the same order, and ends with a declaration. A job application form is shorter and suits salaried roles, with a CV upload and a written answer. Upform has a free template for each.

Can applicants sign the employment application?

Not with a signature. Upform has no signature field, so the template ends with a required Yes/No declaration that the answers are true and complete, saved with the submission date. If you need a legally binding signature, for example on an offer letter, collect it at that stage with an e-signature tool.

Should an employment application ask for a date of birth or Social Security number?

Usually not. Both add discrimination and data protection risk, and neither helps you decide who to interview. If a role has a legal minimum age, ask a Yes/No question about meeting it instead, and collect ID numbers at hire through payroll. Rules differ by country, state and city, so check the law where you hire.

Can people apply from their phone?

Yes. The form shows one question per screen, with full-width answer buttons and a sticky back and next bar on mobile. The resume upload is optional, which matters for applicants who do not keep a resume on their phone.

Where do the applications go after someone submits?

Into the responses table for that form, with search, a date filter, CSV export and a drawer showing each applicant's full set of answers. From there each application can go to Google Sheets, Slack, Notion, monday.com, a webhook or an n8n workflow, each connected with your own account.
