Launch waitlists
Ask for an email, a size and which colorway they are waiting for, one question at a time. Everyone lands on the launch list in Klaviyo, and the answers tell the buying team what to reorder.
Waitlist and registration formsKlaviyo's own sign-up forms are built to catch an email on your storefront. Upform is for the longer asks: a waitlist with a few qualifying questions, a back-in-stock request, a post-purchase survey. Connect your own Klaviyo account once, pick a list, and every submitted response subscribes that person, with their email, name and phone on the profile. It costs nothing on the free plan.
To send a form to Klaviyo with Upform, open the form's Connect panel, click Connect Klaviyo and authorize your own account in the pop-up. Choose a list, map one question to Email (required) and, if the form asks for them, to First name, Last name and Phone. Click Enable for this form, send a test profile, and publish. From then on every submitted response subscribes that person to the list. The integration, all 21 question types and unlimited responses are on the free plan.

Being straight with you
Klaviyo already ships forms, and for storefront capture they are hard to beat. Upform does a different job: it asks the questions a popup cannot, then hands the person to Klaviyo. Here is where each one is ahead.
| Feature | Klaviyo sign-up forms | |
|---|---|---|
| Built for | Longer asks: waitlists, requests and surveys, one question per screen | Fast email and SMS capture on your storefront |
| Formats | Public link, iframe embed or email snippet | Popup, flyout, full page and embed |
| Question types | 21, including picture choice, star rating, slider and file upload | Fields built around profile data and consent |
| Logic | 20 operators and four actions: show, hide, jump, end form | Multi-step forms, a few questions per step |
| On-site targeting and timing | Not available: Upform has no popups or display rules | Built in |
| A/B testing | Not available | Up to seven variations per test |
| SMS consent | No SMS consent step: Phone is mapped as a profile field | Collects consent for email and SMS |
| Every answer, with drop-off | Responses table, CSV export and a per-question drop-off funnel | Form analytics, with per-step engagement on multi-step forms |
| Cost | $0 for the integration on the free plan | Included with your Klaviyo account |
Klaviyo's sign-up form features checked September 2026 in the Klaviyo Help Center; Upform's on our own pricing page. Products change, so verify both before you decide.
How it works
No private API key to paste, no Zap in the middle and no per-task bill. The whole setup lives in the Connect panel of the form you already built.
Open the form, go to the Connect panel and click Connect Klaviyo. A pop-up asks you to authorize Upform against your own Klaviyo account over OAuth. If nothing opens, allow pop-ups for Upform and click again.
Upform loads the lists in your account. Pick the one this form feeds: a launch waitlist, a restock list, a feedback panel. Each form subscribes people to one list.
Point a question at Email, which is required, and optionally at First name, Last name and Phone. Upform pre-fills a best guess from your email question and from question titles that look like names or phone numbers, so most forms only need a glance.
Leave the Track “Upform Submission” event switch on to send a custom event with all answers to the profile. Turn it off if the list subscription is all you need.
Click Enable for this form, then Send test to push a test profile into your Klaviyo before anyone real fills it in. Publish on the public link or as an embed, and every submitted response joins the list.

The result
On the left, what the respondent sees. On the right, the profile waiting in your Klaviyo account, already on the list.

One email when the drop goes live. Nothing else.
An email question, one of 21 types, free on every plan.
Klaviyo · Profile
Example profile. Four questions mapped, one list joined, and the full set of answers carried on the event.
The mapping
There are four profile fields to fill, and Upform's question types line up with them. You confirm each pair once, per form.
| Klaviyo profile field | Upform question to map | What arrives |
|---|---|---|
| Email (required) | An Email question, which validates the address | maya@northgate.io. The connection will not save until this is mapped. |
| First name | A Short Text question titled First name | Maya |
| Last name | A Short Text question titled Last name | Okonkwo |
| Phone | A Phone Number question, with its country flag and dial code picker | +44 7700 900461 |
| Everything else | Any other question in the form | Stays in Upform, and travels on the “Upform Submission” event when that switch is on |
One thing to plan for: Upform's Full Name question collects a name on a single line, while Klaviyo keeps first and last name apart. If you want both fields filled cleanly, ask them as two Short Text questions. Every answer, mapped or not, also stays in Upform's responses table, and the respondent shows up in the Contacts directory.
Use it with
A list is only as useful as what you know about the people on it.
Ask for an email, a size and which colorway they are waiting for, one question at a time. Everyone lands on the launch list in Klaviyo, and the answers tell the buying team what to reorder.
Waitlist and registration formsA picture choice question shows the sold-out products, a dropdown takes the size, and the email subscribes them to the restock list. With the event switch on, their picks sit on the profile too.
All 21 question typesA star rating and one open question after delivery. Customers who answer join a feedback list you can thank properly, and the drop-off funnel shows which question lost the rest.
Customer feedback formsCame here looking for a Typeform Klaviyo integration or a way to run Klaviyo surveys? Upform connects to Klaviyo natively, on the free plan, with no cap on responses. The free Typeform alternative page covers the rest of the switch.
Honest limits
Better to know now than after the launch email has gone out.
Because every submission joins the list, say what people are signing up for before they type their address. A Statement screen or the help text under the email question can spell out what you will send and how often. Make the email question required so every response arrives with one, and use conditional logic to show each person only the questions that apply to them.
Beyond the list
Destinations stack on one form, so one submission can join the Klaviyo list, create a CRM contact and add a spreadsheet row, all on the free plan.
Every destination connects with your own account. Post new signups to a channel with Slack, or browse the full integrations directory.
Use Klaviyo's sign-up forms for a popup, flyout or embed that catches an email on your storefront, with display rules and A/B tests built in. Use Upform when the form has more to ask: a waitlist with qualifying questions, a back-in-stock request, a post-purchase survey, one question per screen with branching logic. Plenty of stores run both, with Klaviyo on the storefront and Upform on the longer forms.
Yours. The connection is per-user OAuth: you authorize Upform against your own Klaviyo account in a pop-up, and the list picker shows the lists in that account. There is no private API key to paste and no shared service account. Disconnect whenever you like and your forms keep collecting responses in Upform. They simply stop reaching Klaviyo.
Four: Email, First name, Last name and Phone. Email is required, and the connection will not save until a question is mapped to it and a list is selected. Upform pre-fills a guess from your email question and from question titles that look like names or phone numbers, and you can change any pair before you enable it.
Yes, if you leave the Track “Upform Submission” event switch on. It sends a custom event with all answers to the profile, so the full response sits in Klaviyo next to the person who gave it. Every answer also stays in Upform, in the responses table with search, filters and CSV export.
No. Every integration is included on the free plan, which is $0 forever and comes with unlimited forms, unlimited responses, all 21 question types and conditional logic. Pro is $12 a month and adds unlimited file storage, removes Upform branding and allows up to five team members. Online checkout is not live yet, so Pro accounts are set up manually and the team replies the same day.
Nothing reaches Klaviyo. The subscription happens when a response is submitted, so someone who opens the form and leaves at question three is never added to your list. They are still counted in Upform analytics as an incomplete, and the per-question drop-off funnel shows the exact question they stopped on.
