Keep pulse checks anonymous
Leave out name and email questions, and list only groups of five or more in any team question. Each response still shows when it arrived and the sender's time zone, so tell people up front what you will see.
Eleven survey templates for customers, teams, classrooms and research. Each one is a finished conversational survey with the questions written and the follow-up logic wired where it helps, and every one collects unlimited responses on the free plan.

A survey template saves the slow part of survey work: deciding what to ask, in what order, and on which scale. Every template below opens as a conversational form, one question per screen, with any follow-up rules already set in conditional logic. Preview it live, clone it, then change anything.
They also stay free once you send them. The free plan has unlimited surveys, unlimited responses and all 21 question types, so a survey never stops collecting halfway through a quarter. That response cap is usually why people go looking for a free Typeform alternative: Typeform's free plan is widely reported at 10 responses a month.

Customers
Loyalty, satisfaction, product direction and churn. Five surveys, each built around one answer you can track over time.
How likely are you to recommend us?
One 0–10 question, then a follow-up worded for detractors, passives or promoters.
FeedbackHow satisfied were you today?
A star rating, a follow-up for happy and unhappy customers, and one would-you-return question.
FeedbackHow was your experience today?
A rating plus the open question that explains it, sent after an order or a support ticket.
FeedbackWhich feature do you use most?
Which features people rely on, what is missing, and what would make them upgrade.
FeedbackWhat's the main reason you're leaving?
Why someone left, what nearly kept them, and who is worth a follow-up call.
FeedbackSend CSAT after a single moment and NPS on a schedule, and keep each in its own form so the numbers never blend.
Customer feedback surveysTeams
How people feel about their work, and how the work itself went. Keep the first anonymous and the second named.
How supported do you feel this quarter?
An anonymous pulse check on workload, management and direction.
HRHow did this quarter go overall?
A self-assessment first, then manager ratings on a 1–5 slider scale.
HRLeave out name and email questions, and list only groups of five or more in any team question. Each response still shows when it arrived and the sender's time zone, so tell people up front what you will see.
Education
For teachers, course creators and anyone who runs training. Ask at the end of a module, while it is still fresh. There is more for classrooms and training on the surveys for education page.
How clear was this week's material?
Course load, support and campus life, on scales you can chart afterwards.
EducationWould you recommend this course?
Pace, materials and instructor ratings, asked at the end of a module.
EducationHow useful was today's session?
Rating scales for training, workshops and suppliers, ready to average in a spreadsheet.
EducationResearch
When no ready-made survey fits the question, start from a neutral structure and write your own.
Which best describes your work?
A neutral structure you can point at research, screening or intake work.
ResearchProblem discovery, segmentation, concept tests and pricing research, with the questions to ask in each.
Plan a market research surveyFor people who already use your product: what they rely on, what they work around, and what is missing.
Run product researchPick one
Start from the decision the answers will feed, not from the template name.
| You want to know | Start with | The question type to lean on |
|---|---|---|
| Would customers recommend you, quarter after quarter? | NPS survey | Slider Scale from 0 to 10 |
| How did one order, visit or ticket go? | Customer satisfaction survey | Star Rating, then an open why |
| What should you build or fix next? | Product feedback form | Single Choice for the feature used most, then Long Text |
| Why do customers cancel? | Exit survey | Single Choice reason, with Something else as an option |
| How is your team doing? | Employee feedback form | Rating scales, answered anonymously |
| Did a course or session work? | Course feedback form | Rating 1–5 for each aspect |
| Something none of these cover | Questionnaire template | A Single Choice screener first |
If two rows seem to fit, three questions settle it. Who answers? Customers give you a minute, employees need to feel anonymous, and students answer best right after class. What will you change? If no possible answer would change a decision, the survey is not ready to send. Will you repeat it? A survey that goes out every quarter should keep its wording fixed, so this quarter's result compares with the last one. Our guide on how to make a survey goes further on audience and sample size.
Five steps
A template is a strong first draft. Ten minutes of editing makes it yours.
Click Use template. The survey is copied onto your account with its questions, logic and styling, and your edits never touch the original.
Delete every question whose answer nobody will act on. Each extra screen costs you completions.
Swap "us" for your product or course name, and keep scale labels and direction the same from one question to the next.
After deleting or reordering, open the Logic tab and read each rule summary, so no follow-up points at a question you removed.
Set your logo, colors and font in the Design panel, share the link or embed the survey, and send each response to Google Sheets.

How satisfied were you overall?
One star is not at all, five is completely. Press 1 to 5, or pick a star.
The customer satisfaction survey template in the builder: steps on the left, a 1:1 live preview in the middle, the question's settings on the right.
2 →
How clear were this module's explanations?
1 is not clear at all, 5 is very clear.
A Rating question on a 1–5 scale, with both ends spelled out in the help text. Rating counts from 1, so a 0–10 scale such as NPS uses Slider Scale instead.
Survey best practices
Most bad survey data is decided by the wording, long before anyone answers. Five habits prevent most of it.
Yes. Every survey template here is free to clone, edit and publish, and it stays free after you send it. The free plan includes unlimited forms and responses, all 21 question types, conditional logic and every integration. Pro is $12 a month and removes Upform branding, lifts the 100 MB storage limit and adds up to five team members.
No. The free plan has no response cap, monthly or total, so a survey keeps collecting for as long as it is published. That is the main difference from Typeform, whose free plan is widely reported at 10 responses a month.
Yes, as long as you leave out name and email questions: respondents never sign in, so a response only carries a name if someone types one. Each response does show when it was sent and the respondent's time zone and browser language, both in the response view and in the CSV export. On a small or spread-out team that can point at one person, so only ask about groups of five or more and tell people what you will see.
Not inside Upform. There is no scoring or calculation engine, so the Results tab shows views, starts, completion rate and average completion time rather than a score. Send responses to Google Sheets or export a CSV, and one formula gives you the NPS or the mean.
No. There is no matrix grid and no ranking question type. For a matrix, ask each row as its own Rating question, which reads better on a phone anyway. For ranking, ask a Single Choice for the top pick, or a Multiple Choice limited to two or three selections.
Publish it and you get a public link to share anywhere, an iframe embed code for your site or app, and an email snippet (a styled link, because most email clients strip iframes). Every response lands in your Upform responses table and, once connected, in Google Sheets, Slack, Notion or HubSpot.
