Twelve ways teams use Upform

Each page shows the questions to ask, the logic to add and the template to clone. Pick the one closest to your job.

Lead generation

Ask four questions instead of eleven, branch on the answers, and push the lead into HubSpot or a Slack channel the second it lands.

Best for: demand gen teams sending paid traffic to a landing page.

Customer feedback

A rating scale, then an open follow-up that changes depending on the score. One question per screen, so people finish it.

Best for: support and success teams who need more than a star average.

NPS surveys

A 0 to 10 slider scale, then a follow-up that asks promoters and detractors two different questions. Run it every quarter, free.

Best for: teams tracking NPS without buying a survey seat per person.

Client onboarding

The brief, the brand files and the billing details in one link instead of six emails and a shared folder nobody opens.

Best for: studios and consultants kicking off a new engagement.

Recruitment

Applications with CV uploads, screening questions up front, and a rule that ends the form politely when someone is not eligible.

Best for: hiring managers running open roles without an applicant tracking system.

Event registration

RSVPs, dietary notes and session picks, ending on a thank-you screen that redirects straight to your calendar link.

Best for: community and events teams, from a 20-seat dinner upward.

Market research

Long questionnaires that do not feel long: one screen per question, a progress bar, and a drop-off report showing where people quit.

Best for: research teams fielding a study without a panel contract.

Product research

Concept tests, pricing checks and feature votes, using picture choice for the visuals and a cap on how many options anyone may pick.

Best for: product managers validating an idea before it reaches the roadmap.

Education

Course feedback, student surveys and enrollment forms that work on a phone in a corridor, with full-width targets and a sticky next bar.

Best for: teachers, course creators and university departments.

Agencies & freelancers

Creative briefs and intake in your client's colors, fonts and logo. Save the theme once and reuse it on the next account.

Best for: agencies running the same kickoff for every new client.

Healthcare

Patient intake and pre-visit questionnaires with file upload for referrals. Forms stay private until you publish them, with uploads scoped per form.

Best for: clinics and practitioners replacing a printed clipboard.

Nonprofits

Volunteer signups, donation interest and program applications, with every respondent rolled into one contacts directory you can export.

Best for: small teams with no budget line for survey software.

By team

Most people arrive with a job title, not a use case

So here is the other way in. These are the pages each team opens first, and the forms they end up publishing in week one.

  • One workspace, so marketing and support are not paying for two tools
  • Invite up to 5 teammates on Pro with granular permissions per form
  • Save a theme once and every team publishes on-brand without a designer
How team access works

Marketing

Demand capture and the research behind the campaign.

Sales

Qualify the enquiry, then hand over a clean record.

HR & People

Hiring at the front, engagement and reviews after.

Product

Evidence before the roadmap, signal after the release.

Support & Success

Measure the relationship, not just the last ticket.

Operations

Intake that lands in the sheet or board you already run.

Same product, twelve jobs

The limits are identical whichever one you pick

There is no surveys tier, no lead capture add-on and no per-response meter. Every use case on this page gets the whole product, including conditional logic, on the free plan.

  • Unlimited forms and unlimited responses, on Free and on Pro
  • All 21 question types, from star rating to file upload to picture choice
  • 20 logic operators and four actions: show, hide, jump, end form
  • Every integration: Sheets, Slack, Notion, HubSpot, Mailchimp, Klaviyo, monday.com, n8n, webhooks
  • Analytics on every form: completion rate, average time, per-question drop-off

One routing question, three branches. A jump rule sends each answer to a different set of screens, so three use cases can live behind a single link.

Getting started

Three steps, whichever use case you picked

  1. Clone the closest template

Open the template gallery, preview any template as a live form, and copy it to your account in one click. Every template is free, including the ones for hiring, intake and NPS.

  2. Edit it in the builder

Rewrite the questions, drag the steps into order, drop in your logo and colors, and add logic where the path forks. The middle column is the real form at 1:1, with desktop, tablet and mobile toggles.

  3. Publish, connect, then watch the funnel

Share a public link or embed it on your site, then send each response to Google Sheets, Slack or Notion. The drop-off report tells you which question is costing you submissions.

Frequently asked questions

Which solution page should I start with?

Start with the outcome, not the form. If you are collecting demand, read lead generation; if you are measuring how people feel about what you shipped, read customer feedback or NPS surveys. Every solution page ends with a template you can clone in one click, so the fastest route is to open the closest one and edit from there.

Do any of these use cases cost extra?

No. All twelve run on the free plan, which includes unlimited forms, unlimited responses, all 21 question types, conditional logic and every integration. Pro is $12 a month and adds unlimited file storage, removal of Upform branding and up to 5 team members. There is no per-use-case pricing and nothing expires, because the free plan is permanent.

Can one form cover more than one use case?

Yes, and conditional logic is how. Ask what the person came for on the first screen, then use show, hide and jump rules to send them down the right branch, so a sales enquiry and a support complaint can share a form and never see each other's questions. You can also end the form early for anyone who does not qualify.

Is Upform a form builder for teams, or for one person?

Both. One person can build, publish and collect on the free plan, which has no team members. On Pro you invite up to 5 teammates by email with granular permissions, and each invite shows as pending, accepted or declined so you always know who has access to which forms.
