Lead capture forms that qualify while they collect

You already paid for the click. The form decides whether it becomes a name your team can work or a row somebody deletes on Friday. Upform asks three questions, branches on the answers, and pushes the qualified ones into your CRM as they arrive.

The short answer

What a lead capture form actually is

A lead capture form is a short form (usually three to five fields) that collects the minimum information needed to follow up with a prospective customer: who they are, how to reach them, and one detail that tells you whether they are worth reaching.

That last part is what separates it from a contact form. A contact form takes a message. A lead capture form takes a message and a signal. Everything else (company size, budget band, the fourteen properties your CRM admin asked for) belongs in a follow-up conversation or behind conditional logic, not in front of a stranger who arrived forty seconds ago.

Field count

The four-field rule

Ask for four things at most on the first pass: who they are, how to reach them, what they want, and one qualifier. Everything past four is a question you are putting to a stranger before they have a reason to trust you with the answer.

Marketing teams have published the same finding for more than a decade: each field you add costs you submissions, and the drop is steepest on phone number, company size and budget, the three fields most often added by someone who will never look at the form again. The fix is not to give up on qualification. It is to move it, as we cover in form design best practices.

  • Full Name (one field, not a first-name and last-name pair)
  • Email (a dedicated question type with real validation, not a text box)
  • One qualifier (a Single Choice question, answered with the A, B or C key)
  • Everything else (shown by a rule, and only to the people it applies to)

Three screens end to end: email, name, then “What do you need a quote for?”.

Conditional logic

Qualify with logic, not with a longer form

Upform does not score leads. There is no points engine and no grade. What there is: a flat list of rules, each one an AND/OR stack of conditions built from 20 operators, with four actions (show, hide, jump, end form).

So a $50,000 project and a $500 project take two different paths through the same three questions. One is asked about timeline and decision-maker, then lands on a Thank You screen that redirects to a booking page. The other gets the price list and a polite close. Neither of them ever sees a field that does not apply to them, which is the whole argument for a multi-step form over a wall of inputs.

  • 20 operators, including is any of, is between, is before and is not answered
  • Conditions stack with AND/OR, so “enterprise budget and starting this quarter” is one rule
  • Jump an out-of-scope lead straight to the Thank You screen instead of wasting their time
  • Every operator and every action works on the free plan
How conditional logic works
Jump to fieldLogic · Rule 1 of 2

Rule Summary

If “Project budget” is any of $25k–50k, $50k+ And “When do you want to start?” is before 31 Dec 2026 Then show “Who signs off on this?” Then jump to “Book a call”

Jump to fieldLogic · Rule 2 of 2

Rule Summary

If “Project budget” is Under $5k Then hide “Who signs off on this?” Then jump to “Thank you, here's the price list”

Two rules. No scoring model, no spreadsheet of weights, nothing to maintain.

Integrations

Where the lead goes the second it is captured

A lead sitting in a responses table is not a lead. Every integration below is shipped, free, and connected with your own account over OAuth. You authorize Upform against your HubSpot, your Slack, your sheet.

Not on the list? The n8n integration takes a webhook URL and signs every request with an HMAC-SHA256 X-Upform-Signature header, so your workflow can verify the payload without you handing over any credentials. Notion and monday.com connect the same way, from the integrations directory.

Analytics

Find the question that kills your form

Every published form gets a per-question drop-off funnel with color-coded severity, so the weak question names itself instead of staying a theory.

Alongside it: views, starts, submissions, incomplete sessions, completion rate, average completion time, a desktop and mobile split, and a country breakdown. Watch the funnel for a fortnight, cut or soften the worst question, watch it again. That loop is the entire method behind a better form conversion rate, and it is the cheapest lever you have on cost per lead.

How to

Build a qualifying lead form in five steps

Ten minutes from an empty account to a published lead capture form that routes itself.

  1. Start from a template, or from nothing

Clone the quote request form onto your account in one click, or open a blank form and add the first question. Either way you are editing your own copy, not a demo.

  2. Cut it to four fields

Name, Email, the qualifier, and nothing else until a rule says otherwise. If a field is there because someone asked for it in a meeting in 2024, delete it and see whether anybody notices.

  3. Write the qualifier as a choice, not a text box

Single Choice with three or four bands answers faster (one keystroke, A through C) and gives you a value you can filter and branch on later. Open text gives you a sentence nobody can sort.

  4. Add one rule

High intent gets two more questions and a Thank You screen that redirects to your booking page. Everything else jumps straight to the close. One rule is usually enough; you can add the second one next week.

  5. Connect the destination, then publish

Hook up HubSpot, a sheet or a Slack channel, then publish on a public link or embed the form in your landing page. Come back in seven days and read the drop-off funnel.

Frequently asked questions

How many fields should a lead capture form have?

Three to four on the first pass is the range that holds up for cold traffic: a name, an email address, and one question that tells you whether the lead is worth a call. Every field past that costs you submissions, and the cost is steepest on phone number, company size and budget. If you genuinely need twelve answers, ask four of them up front and collect the rest with conditional logic once the contact exists.

What is a good form conversion rate?

It depends far more on where the traffic came from than on the form itself. Warm traffic (an email list, a pricing-page visitor asking for a quote) routinely converts several times better than a cold paid click, which makes published industry averages close to useless as a target. Measure your own baseline in Upform's analytics for two weeks, then change one thing at a time and watch the per-question drop-off funnel move.

What is the difference between a lead capture form and a contact form?

A contact form collects a message. A lead capture form collects a message plus a qualifying signal: a budget band, a timeline, a team size, whatever decides who gets called first. In practice they are the same tool: a contact form becomes a lead capture form the moment you add one well-chosen question and a rule that routes the answer.

Can I send captured leads straight to HubSpot, Mailchimp or Slack?

Yes, and on the free plan. You connect your own HubSpot, Mailchimp, Klaviyo, Slack, Google Sheets, Notion or monday.com account over OAuth, and every submission is pushed as it arrives. HubSpot creates or updates the contact, Slack posts a message, Sheets appends a row. Custom webhooks and the n8n integration cover anything that is not on the list.

Is Upform really free for lead generation, or is there a response cap?

Really free, and there is no cap. The free plan gives you unlimited forms, unlimited responses, all 21 question types, conditional logic and every integration, forever. The trade is Upform branding on your forms and 100 MB of file storage. Pro is $12 a month, a launch price at half the $24 list, and it removes the branding, lifts the storage limit and adds up to five team members.

How do I put a lead capture form on my landing page?

Publish the form and you get both a public link and an embed snippet. Drop the iframe into your landing page and the form runs inline with its keyboard shortcuts intact, or point an ad, a QR code or an email button at the hosted page. Both routes work on the free plan, and public submissions are protected by invisible reCAPTCHA v3.
