What's in the template

Eight questions, three for a guest who says no

Each question uses the type that fits it, so every answer lands in your table ready to count. Questions 3 to 7 only appear for guests who said yes.

  1. What's your name? Full Name

First, so every reply can be ticked off your guest list, declines included.

  2. Will you be joining us? Yes/No

The question the form hangs on. Guests tap, or press Y or N. The reply-by date sits underneath.

  3. How many of you are coming, including you? Single Choice

Just me, Two of us, Three of us or Four of us. Fixed options keep the headcount clean.

  4. Who's coming with you? Short Text

Names for the seating plan. Shown only for parties bigger than one.

  5. Choose your main course Picture Choice

A photo per dish instead of a menu to read.

  6. Any allergies or dietary needs? Short Text (optional)

A text box, because "tree nuts, not peanuts" does not fit in a checkbox.

  7. Where should we send updates? Email

For directions and last-minute changes. Validated as an address.

  8. Anything else you'd like to tell us? Long Text (optional)

A song request, a note for the hosts, or regrets from anyone who said no.

  9. Thank you, your RSVP is in Thank You

The closing screen for every reply, with an optional redirect.

What to ask on an RSVP form

An RSVP form has one job: turn an invitation into numbers you can plan around. Every question should feed the headcount, the seating plan or the catering order. Anything else gives a busy guest one more reason to put the reply off.

That leaves a short list: who is replying, yes or no, how many seats and whose names, the main course, any allergies, an email for updates and one open box for everything else. The question types do the checking. The email is validated, and the headcount is a fixed set of options, so nobody types "2ish". Leave off a phone number unless you will really call, and skip the postal address you already used for the invitation. Our form design best practices explain when a question earns its screen.

RSVP wording for weddings, parties and company events

  • Weddings. Match question 3 to the invitation: if it says "you and a guest", keep Just me and Two of us and delete the rest. Keep the song request, the one question guests enjoy. Upload the invitation artwork as the background image and set your colors and font in the design panel, so the reply looks like part of the stationery.
  • Birthdays and dinner parties. Not capping numbers? Replace question 3 with a Number question and set the names rule to Greater than 1. Drop the meal step for a buffet, but keep the allergy question.
  • Company events. Rename question 7 to Work email and add a Single Choice for office or team. Swap the song request for "Anything we should know to make the day work for you?", which is where access needs show up. For a conference open to the public, start from the event registration form template instead; our event registration page explains where an RSVP ends and a registration begins.

Set a reply-by date, then close the RSVP form

Put the date on the invitation and in the description under question 2. Three to four weeks before a wedding gives the caterer and the seating plan room to work; a week is plenty for a party.

When the date passes, unpublish the form. The same link now shows a Form Not Published notice and takes no more replies, and every reply you collected stays in the responses table. A late guest with a good excuse? Publish again for an hour. To chase missing replies, compare your guest list with a CSV export or the Google Sheets integration.

Word the Thank You screen for both answers A yes and a no end on the same screen. "Thank you, your RSVP is in" works for both; "We can't wait to see you!" does not. A redirect can send guests on to your registry.

Two rules that keep a no short and a plus-one named

Conditional logic on questions 2 and 3 ships switched on. Clone the template and the branching is live.

  • If "Will you be joining us?" is exactly No, jump to question 8, the optional message. A decline takes three questions.
  • If "How many of you are coming?" is none of "Just me", show "Who's coming with you?", so every plus-one has a name for the seating plan.
  • Solo guests skip the names question, so their form is seven questions long.
  • Add your own with 20 operators and four actions: show, hide, jump to a question, or end the form.

Question 5

Meal choices guests can actually see

A list of dish names asks guests to imagine dinner. Picture Choice shows it, with a photo per option.

Guests can tap more than one photo, so a couple can pick two different mains in one reply, but the question records which dishes were chosen, not how many of each. For exact plate counts, duplicate question 5 once per seat and show each copy with a rule on question 3.

Allergies stay in a plain text box on purpose: a checkbox turns a serious one into a single tick. The survey question types guide covers all 21.

Share the RSVP by link, QR code or email

Publishing gives the form a public link, an iframe snippet and an email button, all on the Share tab.

  • A link. For a group chat, a text or a calendar invite. On a phone it opens full screen, with back and next pinned to the bottom.
  • A QR code. Upform does not make QR codes, but any QR code generator turns the public link into one for a printed invitation. Test it on two phones before printing.
  • Your event website. Paste the iframe snippet into an embed block. Our guide on how to embed a form covers the common site builders.
  • An email. Most inboxes block iframes, so use the button snippet instead.

Frequently asked questions

Is this RSVP form template free?

Yes, and it stays free. The free plan gives you unlimited forms and responses, all 21 question types, conditional logic and every integration, with 100 MB of file storage. Your form carries a small Upform mark. Pro is $12 a month and removes it, lifts the storage cap and adds up to five team members.

What happens when a guest says no?

They answer three questions at most. A logic rule on question 2 jumps anyone who answers No past questions 3 to 7 to the optional message box, then the Thank You screen. The decline is recorded like any other reply, so your table shows who is coming and who is not.

Can I make a QR code for my RSVP form?

Yes. Upform gives every published form a public link but does not generate QR codes itself, so paste the link into any QR code generator and print the result on your invitation. The link stays the same when you edit the questions, so a printed code keeps working.

How do I close RSVPs after the reply-by date?

Unpublish the form. Anyone who opens the link afterwards sees a Form Not Published notice and cannot send a reply, and every reply you already collected stays in your responses table. Publish it again and the same link works.

Can guests pay or buy tickets through the RSVP form?

No. Upform has no payments or checkout, so the form collects the RSVP and nothing else. If there is a ticket price or a deposit, email your own payment link to everyone who said yes. A Thank You redirect to that link also works, but it sends every guest there, decliners included.

How do I count guests and meals for the caterer?

Export the responses as a CSV or connect Google Sheets and count there. Upform shows every answer in a searchable table but does not count answers per option, so a COUNTIF on the meal column gives the caterer their numbers. The fixed headcount options keep that count clean.
