Every question on a volunteer form should change who you call, when, or for what. If an answer changes none of those, cut the question.
Start with the role, not the paperwork
People arrive knowing what they want to do, not their emergency contact's number. Opening with the roles makes the first tap easy and lets logic shape the rest of the form. Multiple Choice takes an optional maximum: set it to two and you get a real preference, not every box ticked. More on question order in our form design best practices.
Split days from times of day
Availability is where most volunteer sign up forms go wrong. A free text box returns answers like “most weekends, depends” that nobody can plan a shift around. Two Multiple Choice questions, one for days and one for mornings, afternoons or evenings, give you two clean columns to filter. Upform has no grid question, and a 21-box grid is hard work on a phone anyway. The tradeoff: someone free Monday mornings and Saturday afternoons reads as free at both times on both days, so confirm the exact shift when you first get in touch.
Ask about experience in one open question
One Long Text replaces a page of checkboxes. Word the help text around your roles (first aid, working with teenagers, languages spoken) and leave it optional, so a first-time volunteer with nothing to write does not stall.
Keep the driving questions for drivers
Nobody sorting cans needs a question about a driver's license, so questions 2 and 3 appear only for drivers. Use the same pattern for any role with its own requirements: one question per requirement, one show rule per role.
Ask for an emergency contact, and say why
Questions 12 and 13 use a Full Name and a Phone Number with a country flag and dial code, so a number from abroad still works. Say in the help text that you will only use it if something goes wrong on a shift. If some roles are fully remote, add a hide rule so those volunteers skip both screens. An emergency contact is also personal data about someone who never saw your form, so keep it only as long as the person volunteers with you. More in our guide to GDPR compliant forms.
End with how they heard about you
Question 14 is a Single Choice and the only answer here that tells you where to spend next month's outreach.
