Most form templates are a list of field labels. You paste them into a builder, and then you still have to decide what the form asks first, which questions only apply to some people, and what happens when someone says no. That part is the slow part.

These are built as finished forms. Each one opens as a real conversational form (one question per screen, keyboard shortcuts on the choices, a progress bar) with the branching rules already set up. Preview one live before you take it, then clone it onto your account and start editing. Nothing here is a paid pack, and nothing is watered down: the free plan carries all 21 question types, the full logic engine and all nine integrations, which is the whole reason people use Upform as their free Typeform alternative.

Every card opens a live preview. Question counts are what ships in the template before you edit it.

Five steps

From template to a live link in about ten minutes

Nothing here needs a setup call, an import file or a developer. You pick, you edit, you publish.

  1. Pick a template

Filter the gallery by category or search it by name. The card shows the question count before you commit.

  2. Preview it live

Open the real published form, not a screenshot, and answer a couple of questions to see how it moves.

  3. Click Use template

It is cloned onto your account as your own form, with the questions, the logic and the styling intact.

  4. Edit and brand it

Rewrite the questions, drag steps into a new order, then set your logo, colors, Google font and radius in the Design panel.

  5. Publish and connect it

Share the public link or embed it on your site, and pipe responses to Sheets, Slack, Notion or HubSpot.

Wired, not listed

A template with the branching already built

Field lists are easy to copy. The part that takes an afternoon is deciding what happens after each answer, and that is the part these templates ship with.

  • Real rules, not placeholders. Show, hide, jump and end form are already set on the questions that need them, using the same 20 operators you get in conditional logic.
  • The right question type per field. Phone numbers get a country flag and dial code, dates get a picker, ratings get keyboard shortcuts. No text box pretending to be a scale.
  • Copy written for a human. Question text, help text and the thank-you screen are written out, so you are editing sentences rather than inventing them.
  • Analytics from the first response. Completion rate and the per-question drop-off funnel start filling in the moment the form is live, so you can see which borrowed question to cut.

Other places to start

Broader starting points when you know the shape of the form but not the questions yet.

If none of them is close enough, start from a blank form instead. The form builder takes about the same time once you know what you are asking, and our guide to survey questions has the wording to borrow. For longer research projects, how to make a survey covers sample size, question order and the traps that ruin the data before you collect it.

Frequently asked questions

Are the templates free?

Yes. Every template in the gallery is free to use and stays free after you publish it. There is no template tier and no paid pack: the free plan includes all 21 question types, the full logic engine and every integration, so nothing inside a template is gated. The only thing the free plan holds back is removing Upform branding from your published form, which is $12 a month on Pro.

Can I edit a template?

Completely. Using a template clones it onto your account as an ordinary form, so you can rewrite every question, add or delete steps, drag them into a new order, rewire the logic rules and restyle it with your own logo, colors and font. Nothing is locked. Edits to your copy never touch the original template, so you can clone the same one again for the next client.

How many form templates are there?

There are 24 templates in the gallery today, across nine categories: lead generation, feedback, HR, events, onboarding, education, healthcare, nonprofit and ecommerce. Inside the app the same gallery has category chips and a search box, so you can find one by name rather than scrolling. We add templates as people ask for them.

Do the templates include conditional logic?

The ones that need it, yes. The RSVP template only asks about dietary requirements after someone says they are coming, and the NPS template sends promoters and detractors to different follow-up questions. Logic is part of the free plan, so you get all 20 operators and all four actions, and you can change or delete any rule that shipped with the template.

Can I use a template for client work?

Yes. There is no attribution requirement and no restriction on commercial use, which is why agencies and freelancers use the intake, brief and feedback templates as their standard starting point. On Pro you can also remove Upform branding and invite up to five team members, which is usually what client-facing work needs.
