Client intake forms that replace the kickoff email thread

One link collects the scope, the budget range, the deadline, the decision makers and the brand files before your first call. Upform asks one question per screen, branches by service line, and takes unlimited intakes on the free plan.

Unlimited forms and unlimited intakes on the free plan. Forever.

What a client intake form is

A client intake form is a structured questionnaire a new client completes before work starts. It collects the facts a project cannot begin without (contact details, scope, goals, budget range, timeline, decision makers and existing assets) in one pass, so the brief arrives complete instead of arriving across fourteen replies.

The kickoff email thread is not a bad process because clients are disorganized. It is bad because it has no structure. Every question you forget to ask becomes a reply two days later, every answer sits in a different message, and the logo always arrives last.

An intake form is not a proposal, a contract or a discovery call. It is the thing that makes the discovery call short. Send it once the lead has qualified, and the kickoff changes from tell me about your business into we read your answers, here is what we would do.

Intake form, onboarding form, client questionnaire Three names for overlapping documents. Onboarding means the whole sequence: intake, contract, invoice, kickoff. A client questionnaire is the creative half. The intake form is the part that collects facts, and the part that belongs in a form builder rather than a document.

What belongs on a client intake form

Ten things, and each has to earn its place. The test: if the answer would not change what you do, what you charge or when you start, it is a question for the call. Here is the set that passes, with the Upform question type for each.

  • Contact details. Full Name, Email and Phone Number, which carries a country flag and dial code. You need one named person you can reach; projects stall when the reply goes to an inbox nobody owns.
  • Company and website. Company Name and Website. The URL alone answers half of what you would otherwise ask about industry, size and tone, and it costs two seconds to paste.
  • Project scope. A Long Text question: what do you want made? Ask it before your vocabulary leaks into theirs, because this is the answer you will quote back in the proposal.
  • Goals. A Multiple Choice shortlist (more leads, fewer support tickets, a launch, a rebrand), then a Long Text. The list makes it easy to start; the box is where the useful sentence turns up.
  • Budget range. A Single Choice with bands, never an open Number field. A band gets answered because choosing a range is not a commitment, and it saves a meeting neither of you wanted.
  • Timeline. A Date field for the deadline that matters and a Single Choice for how firm it is. Before the trade show and sometime this quarter are different projects at different prices.
  • Decision makers. A Short Text for who signs off and a Number for how many people review. Ask it here and you will not meet a fourth reviewer in week five.
  • Existing assets. A File Upload for logos, brand guidelines, copy documents and photography. Collecting them now is the difference between starting Monday and starting whenever the files turn up.
  • Brand references. A Website field for three sites they admire, or a Picture Choice grid to steer the answer. Taste is easier to show than describe, and this question prevents most of the second revision round.
  • Success criteria. One Long Text: how will we know this worked? Clients enjoy answering it, most intake forms leave it out, and the answer becomes the first line of the brief.

With sub-fields counted, that is eleven to fourteen screens. On one scrolling page it looks like paperwork. As a conversational form it is a short sequence with a progress bar, and most clients finish it on a phone the evening it lands.

Order matters more than length Name, company and website first, then scope, then budget, uploads last. People finish what they have already started, so the questions that take thought belong in the middle and the file picker belongs at the end, once the form has earned the effort. Our guide to form abandonment has the numbers.

The budget question

Ask about budget in a way people answer

Budget is the question intake forms skip most often, and skipping it is why the proposal comes back to a number nobody expected. Bands fix it: the client picks a range on a screen of its own, rather than negotiating over email.

  • Single Choice with A–E keyboard shortcuts. One keypress, no blank box to stare at.
  • Always include a not-sure-yet option, so nobody abandons the form on this screen.
  • Branch on the answer: a band below your minimum ends the form on a screen pointing at your packages.
  • The band lands in the response row, so the pipeline sorts itself before you open it.

Branching

One intake form, every service line

Studios end up maintaining four intake forms because a brand questionnaire and a web build questionnaire share only their first three questions. Ask the service line up front and let one form sort itself out.

  • 20 operators, including is any of, is none of, contains, greater than, between and is uploaded.
  • Four actions: show a question, hide it, jump ahead, or end the form on a screen you wrote.
  • Each rule is a flat AND/OR group, readable out loud before you publish.
  • One link in your signature covers every service you sell, with answers in one table.

Logic routes, it does not calculate. There is no scoring engine, so pricing still happens in the proposal. See forms for agencies for the rest of the process.

Assets

Collect the brief and the files together

Most of the kickoff thread exists to chase attachments. A File Upload question ends it: the client attaches what they have while still answering, with no account, no login and no request for drive access.

Logos and brand guidelines

Set the question to accept SVG, PDF, AI or PNG and cap the size. What arrives is the file you asked for, not a logo screenshotted out of a slide deck.

Copy, content and spreadsheets

Existing page copy, a product list, last year's report. One upload question replaces the three-message exchange about where the content lives and who owns the folder.

References and photography

Screenshots of sites they like, product shots, the deck their board already approved. Pair the upload with a Picture Choice grid when you would rather offer three directions than read a paragraph.

Uploads are scoped to the form that collected them and open from the response drawer alongside the answers. The free plan includes 100 MB of storage; Pro removes the cap.

How file uploads work

After the submit

Where intakes land

Every intake sits in the responses table with search, a date filter, CSV export and a side drawer holding the full answer set and the files. Then it goes where your projects already start.

Notion only sees what you share

The Notion connection lists only the databases you have explicitly shared, so a client tracker syncs without handing over the rest of your workspace.

A client directory you didn't build

Everyone who submits appears in Contacts with their name, email, company, submission count and when you last heard from them.

Your accounts, not ours

Every integration connects through your own account over OAuth, and n8n and custom webhooks sign each delivery with an HMAC-SHA256 header.

Frequently asked questions

What should a client intake form include?

Ten things: contact details, company and website, project scope, goals, budget range, timeline, decision makers, existing assets, brand references and success criteria. Anything that would not change what you do, what you charge or when you start belongs in the kickoff call instead. Ten questions is around twelve screens in a conversational form, which most clients finish on a phone in under five minutes.

Is the client intake form free?

Yes, and it stays free. The free plan includes unlimited forms, unlimited intakes, all 21 question types, conditional logic, every integration and 100 MB of file storage. Your forms carry a small Upform mark. Pro is 12 dollars a month and removes the mark, lifts the storage cap and adds up to five team members.

Should I ask about budget on a client intake form?

Yes, but ask it as a Single Choice with bands rather than an open number field. A band gets answered because picking a range is not a commitment, while a blank box reads like a negotiation and gets skipped. Add a not-sure-yet option so nobody abandons the form on that screen, and put the question after scope, once the client has already invested a few answers.

Can clients upload logos and brand files in the form?

Yes. Add a File Upload question, choose which file types you accept and set a maximum size. Clients attach files inside the form with no account, no login and no shared drive invitation. Uploads are stored scoped to that one form and open from the response drawer, and the free plan includes 100 MB of storage.

Can one intake form cover several service lines?

Yes, and that is the usual setup. Ask which service the client is here for as a Single Choice or Dropdown near the top, then use conditional logic to show only the questions that service needs. One link covers web, brand and retainer work, and every answer still lands in the same responses table with the service recorded against it.

Where do client intakes go after someone submits?

Every intake lands in the responses table for that form, with search, a date filter, CSV export and a side drawer holding the full answer set and any uploaded files. From there it can create a Notion database row, a Google Sheets row, a Slack message, a monday.com board item, a HubSpot contact, a custom webhook call or an n8n workflow run. Each integration connects with your own account over OAuth.
