What this integration costs

The short answer

One database, one mapping, then it runs itself

You authorize Notion from the Connect panel, choose a database, and tell Upform which question feeds which property. From then on every submission creates a row. Answers land as the property type you picked, not as a wall of text in a single cell, so you can filter, group and sort the moment the row appears.

The one thing that stops people on day one is Notion's permission model. A database shows up in Upform's picker only if you have explicitly shared it with the Upform connection. That is a Notion rule, it applies to every integration Notion has, and it takes about ten seconds to fix. Jump to the fix if that is why you are here.

Setup

How to connect a form to Notion

Five steps, once per form. Nothing to install and no automation service in the middle.

  1. Build the form

Add your questions in the form builder, or clone a ready-made one such as the client intake form. Question types matter here: a Single Choice question can become a Notion select, while the same options typed into a Short Text field cannot.

  2. Open Connect and choose Notion

The right panel of the builder switches between Question settings, Design, Logic, Connect, Share and Results. Open Connect, find Notion in the destination list and press connect.

  3. Authorize, and tick the pages you want

Notion shows its own consent screen with a page picker. Whatever you tick there is the complete list Upform will ever be able to see or write to. Tick the database itself, or the page that contains it. This is the step people skip.

  4. Pick the database and map the fields

Choose one database, then match each question to a property. Upform reads the property types straight from your database, so you are picking from the real schema rather than typing column names and hoping.

  5. Save, publish, send one test response

Saving is manual, and Upform warns you if you try to leave with unsaved changes. Publish the form, answer it once yourself, and check the row. Then watch the completion rate and drop-off in Results as real answers come in.

Troubleshooting

Why your Notion database isn't showing up in the picker

Nine times out of ten the database exists, the connection works, and the list is still empty. Notion integrations are deny-by-default: an integration can only see pages and databases a human has handed to it. Nothing is broken. The share step just hasn't happened yet.

The picker only lists what you shared. In Notion, open the database as a full page, click the ••• menu in the top right, choose Connections, and add Upform. Confirm when Notion asks. Go back to the Connect panel, reload the database list, and it will be there.

Five things to check, in order

  1. You clicked through the page picker

If you pressed Allow without selecting anything, the connection is valid and empty. Disconnect, connect again, and use Notion's Select pages step properly.

  2. The database is inline, not a page

An inline database inside a document is not shareable on its own. Share the parent page instead, or open the database as a full page first.

  3. You are looking at a linked view

Linked views point at a source database that may live elsewhere. Open the source (the ••• menu shows the original) and share that.

  4. It is in another workspace

If you belong to more than one Notion workspace, the connection is tied to the one you authorized. Check the workspace name on Notion's consent screen.

  5. Your own access is limited

You cannot share a database with an integration if you only have comment or view access. Ask the workspace owner for full access, or have them add the connection.

Field mapping

Which question type becomes which Notion property

Upform reads your database schema and offers only the properties that can hold the answer. Here is the mapping people use most.

Upform questionNotion propertyWhat lands in the row
Full Name, Short TextTitleThe row's name, so the record is readable in every view
Long Text, Address, Company NameRich textThe full answer, line breaks intact
EmailEmailA clickable address you can filter and dedupe on
Phone NumberPhoneThe number with the country dial code the respondent picked
WebsiteURLA live link, ready for a rollup or a button
Number, Rating, Star Rating, Slider ScaleNumberA numeric value you can sum, average or chart
Single Choice, Dropdown, Yes/NoSelectOne option, matched to an existing select value
Multiple Choice, Picture ChoiceMulti-selectEvery option the person chose, as separate tags
DateDateA real date, so calendar and timeline views work
File UploadFiles & mediaA link to the uploaded file, scoped to that form

Notion property types checked September 2026 against Notion's public API documentation. Notion adds property types over time, so open your database and confirm the schema before you map.

Front end

Ask it like a conversation. Store it like a database.

Respondents see one question per screen, answer with the keyboard and never meet a grid of properties. Notion gets clean, typed data anyway.

  • A, B and C shortcuts on choices; Y and N on yes-no; Enter to continue
  • Conditional logic with 20 operators, so nobody answers an irrelevant question
  • Your logo, colors, font and background, so the form never looks borrowed
  • Full-width targets and a sticky back/next bar on phones

Which services do you need?

Pick up to three. Each one becomes a tag on your Notion row.

A Brand identity
B Website design
C Motion & video
D Ongoing retainer
Next →press Enter ↵

Multiple Choice → Multi-select. Two tags land on the row: Brand identity, Website design.

What Notion forms can't do that Upform can

Notion ships its own form feature, and for plenty of jobs it is the right call. Here is the honest split.

Use Notion's own form when

  • It is an internal request that only teammates will fill in
  • You want zero setup and the database is already open in front of you
  • The questions are short, flat and the same for everybody
  • Nobody will ever ask why half the people abandoned it

Use Upform when the form faces the outside world

  • Branching: 20 operators that show, hide, jump or end the form
  • A one-question-per-screen runtime built for strangers on a phone
  • Drop-off analytics that name the question losing you answers
  • One response in several places at once: Notion, Slack and Google Sheets
  • Design control, invisible reCAPTCHA v3 and a respondent directory

Notion's form feature and Upform's feature set checked September 2026. Both products ship changes often, so verify on the current product pages before you decide.

Migrating from somewhere else? Upform also does a one-way Google Forms import, and the Typeform alternative page covers the switch most people are actually making.

Frequently asked questions

Does the Notion integration cost anything?

No. Every integration is on the free plan, which is $0 forever with unlimited forms and unlimited responses. There is no task quota and no per-row charge, because Upform writes to Notion directly rather than through an automation service you also pay for. Pro at $12 a month adds unlimited file storage, removes Upform branding and lets you invite up to five teammates.

Why can't I see my Notion database in the picker?

Because Notion only shows an integration the pages and databases you explicitly share with it. During authorization Notion displays its own page picker, and whatever you tick there is the complete list Upform will ever see. Open the database in Notion, use the ••• menu in the top right, choose Connections, and add Upform. Then reload the picker in the Connect panel and it will appear.

Can one form write to more than one Notion database?

Each form writes to one Notion database, chosen in the Connect panel. You can still fan the same response out to other places at the same time: Google Sheets, Slack, HubSpot, Mailchimp, Klaviyo, monday.com, n8n and custom webhooks all run alongside Notion on the same form. If you need two Notion databases, connect a webhook and route the second copy yourself.

What happens to file uploads?

A File Upload question maps to a Files & media property, and the row carries a link to the file stored in Upform. Uploads are scoped per form, so a link from one form cannot be used to browse another. The free plan includes 100 MB of file storage and Pro makes it unlimited.

Is the sync two-way? Can editing the Notion row change the response?

No, it is one-way: Upform creates the row, then the row is yours. Edit it, move it, add properties or run it through a Notion automation and nothing flows back. The original answers stay in the Responses table in Upform, where you can search them, filter by date and export to CSV.

Whose Notion account does the form use, and can I revoke it?

Yours. Every integration is per-user OAuth, so you connect your own Notion workspace and nobody else's credentials are involved. You can remove Upform at any time from the database's Connections menu or from your Notion workspace settings, and new submissions simply stop creating rows while the responses keep landing in Upform.
