What's inside

One score, then the right follow-up

Everything in the survey hangs off question 1. Here is that screen as a respondent sees it.

1 →

How satisfied were you overall?

One star is not at all, five is completely. Press 1 to 5, or pick a star.

★ 1★ 2★ 3★ 4★ 5
Next →press Enter ↵

A Star Rating question, one of 21 types and free on every plan. It carries your logo, colors and font behind a short branded loading overlay. See how conversational forms behave.

The full step list

  1. How satisfied were you overall?

    Star Rating
  2. What worked best for you?

Long Text Shown on 4 or 5
  3. What let you down?

Long Text Shown on 1 to 3
  4. How easy was it to get help when you needed it?

    Rating 1–10
  5. Would you use us again?

    Yes/No
  6. Anything else you want to tell us?

    Long Text Optional
  7. Thank you

    Thank You

Questions 2 and 3 are two halves of one branch, so nobody answers more than five. The Thank You screen is pinned last and can redirect to your help center. Swap any question type, add your own, or drag the order around.

Before you send

How to run a satisfaction survey people finish

A satisfaction survey is not a research project. It is one number you can watch week to week, plus enough context to know what moved it. Six questions sent the day something happened beat a twenty-question study sent once a quarter.

CSAT, NPS and CES are not interchangeable

CSAT is the score this template produces: how satisfied someone was with one interaction, on a 1 to 5 star rating, asked right after it. Your CSAT is the share of people who picked 4 or 5, written as a percentage. NPS asks something else (how likely you are to recommend us, 0 to 10) and measures the relationship rather than the moment, which is why it belongs on a cycle; the NPS survey template covers that one. CES measures effort, and question 4 here is the CES-shaped question. Start with CSAT: it is the cheapest to send, the fastest to read, and it attaches to something your team already does daily. Customer feedback covers running all three together.

Send it while the memory is still sharp

Trigger the send on the interaction, not on a date in the calendar. Within 24 hours of a delivered order, a closed ticket or a finished onboarding call, people can still tell you what actually happened; a week later they rate a general mood instead. Put the link at the end of the message that already goes out, and send once per interaction, with no reminders.

Why the branch beats one "any comments?" box

Most satisfaction surveys end with a neutral open box, and it collects almost nothing. Ask someone who gave five stars what went wrong and they skip it. Ask someone who gave two what went well and you sound like you were not listening. One box has to be worded so blandly that neither of them has a reason to type.

So the template splits it. Four or five stars gets "What worked best for you?", a question about a good thing, which people answer in a sentence and which tells the team what to keep doing. Three or below gets "What let you down?", which is permission to be specific, asked at the one moment someone is still annoyed enough to be precise. Nobody sees both, so the survey is a question shorter for everyone and the written answers are about something.

Ask what you can act on, then stop

Questions 4 and 5 are one tap each, and they sort the responses table into piles you can work with: people who struggled to get help, people who will not come back. Question 6 is optional on purpose: a relief valve most people skip, and a blank there means nothing. Six steps, under a minute, and every question past that costs you completions. Survey questions has wording you can copy for the ones you add.

Close the loop on the thank-you screen

Say what happens next in one sentence: that a person reads every low score, or that you reply within a day. It is the only screen where you get to speak rather than ask, and it decides whether the whole thing felt like a form or like a conversation.

Then watch where people leave Let fifty responses in, then open the per-question drop-off funnel in form analytics. On a survey this short the drop is almost always question 1: if the rating is not the first thing people see, they never start. More on form abandonment.

The branch that ships with the template

Two rules ship switched on, in the same Logic panel every form uses: a flat list of AND/OR conditions, 20 operators, four actions (show, hide, jump and end form). Both rules read the star rating from question 1.

If “Overall satisfaction” greater than 3Then show “What worked best”

2 →

What worked best for you?

One sentence is plenty. It goes to the people who did it.

The driver called ahead and waited while I found my keys.
Next →press Enter ↵

The happy path. Four and five-star answers become the specifics you can repeat, not a wall of "all good, thanks".

If “Overall satisfaction” less than 4Then show “What let you down”

3 →

What let you down?

Be as specific as you like. This one reaches the person who can fix it.

Nobody replied to my second email for four days.
Next →press Enter ↵

The recovery path. Every low score arrives with a reason attached, which is the only version of a bad review you can do anything about.

Edit either rule or add your own: a Star Rating can be tested with equals, greater than, less than, is answered and is not answered, then acted on by showing a question, hiding it, jumping to another step or ending the form. The full operator table is on the conditional logic page.

After the submit

Where the scores land

Every response lands in the table for that form, with search, a date filter, CSV export and a drawer holding one respondent's full answer set. Then push it to where the team already looks.

Each one connects with your own account, so scores land in your sheet, your channel, your board. Browse every destination on the integrations page.

Frequently asked questions

Is this customer satisfaction survey template free?

Yes. Cloning it, publishing it and collecting answers all sit on the Upform free plan, and that plan is permanent, not a trial. There is no response cap, so a survey sent to 50 people and one sent to 50,000 cost the same. Pro is $12 a month and removes the Upform mark, lifts file storage and adds up to five team members.

How is a CSAT score calculated?

Count everyone who picked 4 or 5 stars on question 1, divide by everyone who answered it, and multiply by 100. That percentage is your CSAT score. Export the responses as CSV or push them to Google Sheets and it is one formula. Compare the number against your own last month rather than a published industry average, because timing and sample size move it more than quality does.

What is the difference between CSAT, NPS and CES?

CSAT asks how satisfied someone was with one interaction, on a 1 to 5 scale, straight after it happens. NPS asks how likely they are to recommend you, on a 0 to 10 scale, on a cycle rather than after an event. CES asks how much effort something took, which is question 4 here, rated 1 to 10. Use CSAT for the moment, NPS for the relationship and CES for the process.

Why does the survey ask a different question depending on the score?

Because someone who gave five stars and someone who gave two have nothing useful to say to the same prompt. Two logic rules ship switched on: a score of 4 or higher shows What worked best for you, a score of 3 or lower shows What let you down. Each respondent sees one of them and never both, so the survey stays under a minute and the written answers are worth reading.

When should I send a customer satisfaction survey?

Trigger it on the interaction, not on a date in the calendar. Within 24 hours of a delivered order, a closed ticket or a finished onboarding call, while the detail is still in someone's head. Put the link at the end of the message that already goes out, and send once per interaction with no reminders.

Can I change the star rating and the branding?

All of it. Swap the Star Rating for a Rating out of ten or a Slider Scale, rewrite any question, add or delete steps, and drag the order around. The design panel covers your logo, background image, colors, a Google font, corner radius, alignment and the progress bar style. Removing the Upform mark is the one thing that needs Pro.
