The short answer

To send a form to monday.com with Upform, open the form's Connect panel, authorize your own monday.com account, pick a board and (if you like) a group, choose which answer names the item, then map board columns to questions. Save, send a test item, publish. From then on every submitted response creates one item on that board. Upform adds no item cap and no task quota: every integration ships on the free plan, next to unlimited forms, unlimited responses and all 21 question types. Your monday.com plan's own limits still apply.

How it works

Connect a form to monday.com in five steps

No API token to generate, no automation service in the middle counting tasks. It all happens in the form's Connect panel.

  1. Connect your monday.com account

Open the form, switch the right panel to Connect and press Connect monday.com. A monday.com window asks you to authorize Upform against your own account. When it closes, the panel shows the account name, its monday.com address and your email.

  2. Pick a board and a group

The board list is loaded from your account, so you choose from boards you can already open. The group is optional: leave it on Default group, or send every new item straight into a group such as New requests.

  3. Choose what names the item

Every monday.com item needs a name. Pick the question whose answer should become it, usually Full Name or Company Name. Left on Auto, Upform uses the respondent's email or a timestamp.

  4. Map board columns to questions

Upform reads the board's columns and lists each one with its type. Pair each column with the question that should fill it. Anything left on Not mapped stays empty on the board and complete in Upform.

  5. Save, send a test, publish

Press Enable for this form, then Send test to create a test item on the board. Previewing the form creates nothing, so the test item is how you check the mapping. Then publish, and every submitted response creates an item.

Column mapping

Map board columns to form questions

The mapping is what makes the board usable. Each answer lands in the column built for it, so the board's own views, filters and automations work with it from the first item.

Upform lists the board's mappable columns with each one's monday.com type next to the title, and every question in the form is available beside it. Match like with like: an Email question to an email column, a Phone Number question to a phone column.

Status and dropdown columns carry labels that are defined on the board itself. Send a test item and check those columns before you publish.

What arrives

What a new item on the board contains

The shape is the same for every response, which is what lets a team sort, assign and move items without opening each one.

Column types that never appear in the mapping list: subtasks, auto number, creation log, last updated, button, formula, progress, mirror, dependency and file. monday.com fills most of those itself, and anything collected with file uploads stays in Upform, scoped to its form.

Worth knowing

What the monday.com integration does not do

Four limits to know before you build a workflow on top of it. They apply on every plan.

One board per form

Each form carries one monday.com mapping: one board, one group. To feed two boards, run two forms, or add a custom webhook for the second destination.

Every submission is a new item

The integration creates items, and the setup has no option to find and update an existing one. Somebody who submits twice appears on the board twice, so map an email column to spot repeats.

Some columns are not offered

Calculated and system columns (formula, mirror, progress, auto number and the rest) and file columns are left out of the mapping list. Give every answer you need a plain column to land in.

Only submitted responses create items

Somebody who leaves halfway adds nothing to the board. They are counted in form analytics as incomplete, and the drop-off funnel shows the question where they stopped.

Use it with

Where a board item beats an inbox

An email tells one person something happened. An item on a board gives the request an owner, a status and a place in the queue.

Client requests, triaged on the board

A studio sends new clients a client intake form. Name, company, deadline and the brief land as an item in New requests, where somebody assigns an owner and moves it forward.

Client onboarding forms

Applications as a hiring pipeline

Each answer to a job application form becomes an item with the role, email and phone in their own columns. The CV stays in Upform, one click away in the response drawer, while the board tracks who is at which stage.

Recruitment forms
Logic shapes the form, not the destination

Conditional logic decides which questions a person sees (show, hide, jump and end form), not which board or group the item goes to. The group is set per form, so two intake paths that belong in two groups are two forms, each with its own mapping.

An honest split

When monday.com's own forms are enough, and when they are not

monday.com ships its own form builder, and for plenty of jobs it is the right call. Before you pick a tool for monday forms, decide whether the form stays inside your team or faces people outside it.

Use monday.com's own form when

  • The people filling it in already work on the board
  • You want the form and the board set up in one place
  • The answers only ever need to live on that one board

Use Upform when the form faces the outside world

  • One question per screen, built for strangers on a phone, in conversational forms
  • 20 logic operators that show, hide, jump or end the form
  • Drop-off analytics that name the question losing you answers
  • The same response on the board, in Slack and in a Google Sheets row at once

monday.com's form feature and Upform's feature set checked September 2026. Both products ship changes often, so confirm on the current product pages before you decide.

Beyond the board

Run monday.com next to the rest of your stack

A board is where the work gets tracked. When a person should hear about it right away, or a system should act on it, add another destination to the same form.

Slack, for the heads-up

Post one message per response into the channel the owners read, while the item waits on the board. Slack says it arrived; the board says who owns it.

Form to Slack

Custom webhooks, for everything else

Send the whole response as JSON to any endpoint you control: your own database, an internal tool or an n8n workflow. It runs on the same submission as the monday.com item.

Form webhooks

Every destination connects with your own account and runs on the free plan. The full list lives in the integrations directory.

Frequently asked questions

Do I need a paid plan for the monday.com integration?

No. Every integration is included on the free plan, which is $0 forever and comes with unlimited forms, unlimited responses, all 21 question types and conditional logic. Pro is $12 a month and adds unlimited file storage, removes Upform branding and allows up to five team members. Online checkout is not live yet, so Pro accounts are set up manually and the team replies the same day.

Whose monday.com account does Upform connect to?

Yours. The connection is per-user OAuth: you authorize Upform against your own monday.com account, and the board list is loaded from that account, so you only ever see boards you can already open. There is no shared token and no API key to paste. Disconnect whenever you like and the form keeps collecting responses in Upform; new items simply stop appearing.

Can I choose which group the new item goes into?

Yes. After you pick the board, the Group setting lists that board's groups. Leave it on Default group and monday.com decides, or point it at a group such as New requests so every item starts in the same place. Each form carries one board and one group, so two intake paths that belong in two groups are two forms.

What becomes the name of the monday.com item?

The answer to the question you choose in the Item name setting, usually Full Name or Company Name. If you leave it on Auto, Upform names the item with the respondent's email or a timestamp. Pick something a person can recognize at a glance, because the item name is what the whole team sees in every board view.

Can file uploads go into a monday.com file column?

No. File columns are not offered in the mapping list, so uploaded documents stay in Upform, scoped to the form that collected them. Open the response in the side drawer to download them. Map the respondent's name and email to the board so the item tells you whose files to look for.

What happens if an item fails to reach monday.com?

The response is safe. Upform writes every submission to the form's response table before any integration runs, so a failed item never costs you the answer. The latest error is shown under Form sync in the Connect panel, and Send test lets you check the fix before real responses arrive.

Do incomplete responses create items?

No. An item is created when a response is submitted, so somebody who leaves halfway does not add a half-empty item to the board. They are still counted in Upform analytics as incomplete, and the per-question drop-off funnel shows the exact question where they stopped.
