What's inside

One feature, one score, then the gap

Every answer in the form is tied to a single feature. Question 1 is where the respondent picks it.

Which feature do you use most?

Pick the one you would miss first if it disappeared tomorrow.

A Single Choice question, one of 21 types and free on every plan. Rename the options to your own product areas. See how conversational forms put one question on each screen.

The full step list

  1. Which feature do you use most?

    Single Choice
  2. How well does it do the job for you?

    Rating 1–5
  3. What gets in the way?

Long Text Shown on 1 to 3
  4. What is missing that you expected to find?

    Long Text
  5. Have a screenshot that shows it?

    File Upload Optional
  6. Can we contact you about your answers?

    Yes/No
  7. What's the best email to reach you?

    Email Shown on Yes
  8. Thank you

    Thank You

Question 3 appears only for a score of 3 or lower, and question 7 only after a yes, so a happy user who would rather not be contacted answers five questions. The Thank You screen is pinned last and can redirect back into your app. Swap any question type, add your own, or drag the order around.

How to run a product feedback survey that shapes the roadmap

A product feedback form is not a feature request box. A request box collects solutions ("add a dark mode"), mostly from your loudest users. This template collects the problem behind the request, tied to the feature where it happened and a score you can compare month to month. That is the difference between a wish list and something a product team can prioritize. Product research covers the wider program this form fits into.

Anchor every answer to one feature

Question 1 does the sorting for you. When someone picks the reporting screen, every answer after it is about the reporting screen, so the responses table splits cleanly by feature and nobody has to guess what "it's slow" referred to. Keep the list to four to six of your most used areas plus "Something else". If you have twenty features, run one form per area.

Use a 1 to 5 rating, and keep NPS for the relationship

Question 2 is a Rating question set to five points. Five is enough to separate "works for me" from "gets in my way" without asking people to choose between a 6 and a 7. NPS asks something different, how likely someone is to recommend the whole product on a 0 to 10 scale, and it belongs on a quarterly cycle; the NPS survey template is built for that. Run both, just not in the same form. Rating scale questions goes deeper on picking a scale.

Ask what is missing, not what to build

Question 4 reads "What is missing that you expected to find?" on purpose. "What should we build next?" invites people to design your product for you, and the answers come back as features. "What did you expect to find" gets you the job they were trying to do when they hit the wall, which is the part you can use. Read twenty of these and the same three or four gaps start to repeat. Those are your roadmap candidates, each one with a feature and a score attached.

Keep the screenshot optional

A screenshot shows in two seconds what a paragraph takes a minute to describe: the error message, the broken table, the setting someone could not find. It is also the question most likely to stall someone on a phone, so it is optional and says so. Set the allowed file types and a maximum file size in the question settings, so a long screen recording does not arrive where you wanted a screenshot. Uploads count toward the 100 MB of file storage on the free plan; Pro has no storage limit. More on file uploads.

Ask permission before you follow up

The most useful product feedback turns into a fifteen-minute call. Question 6 asks for permission first, and only a yes opens the email field, so you hold contact details for the people who agreed to hear from you and nobody else. It also makes consent easier to show; the GDPR-compliant forms guide covers the rest.

Send it where people use the product

Feedback about a feature is best collected near the feature. Put the link in your in-app help menu, in the email that announces a release, or at the end of onboarding once someone has had a week with the product. Share the public link or embed the form on a page from the share and embed panel. Ask each person once a quarter at most: a survey that turns up every week teaches people to ignore it.

Then read the totals, not only the answers Once 50 responses are in, open the Results tab. Views, starts, submissions and completion rate tell you whether the people who open the form finish it. If starts are high and submissions are low, the form is probably too long for where you placed it. Form analytics explains each number, and form abandonment covers what to cut first.

The two rules that ship with the template

Both live in the same Logic panel every form uses: a flat list of rules, each an AND/OR set of conditions, with 20 operators and four actions (show, hide, jump and end form).

The recovery path. A rating of 1, 2 or 3 opens this screen, so every low score arrives with the reason attached and a 4 or 5 moves straight on to what is missing.

Edit either rule or add your own. A Rating question can be tested with equals, greater than and less than; a Yes/No with is exactly, is any of, is none of, is answered and is not answered. The full operator table is on the conditional logic page.

After the submit

Where the feedback lands

Every response lands in the table for that form, with search, a date filter, CSV export and a drawer holding one respondent's full answer set, uploads included. Then send it to where the product team already works.

Slack, Notion, HubSpot, Google Sheets and monday.com connect with your own account; n8n needs only a webhook URL. Slack posts every response, not only the low scores, and each form posts into one channel; to alert on low ratings alone, send responses to n8n and filter there. Notion lists only the databases you explicitly share with the connection. Browse every destination on the integrations page.

Frequently asked questions

Is this product feedback form template free?

Yes. Cloning it, publishing it and collecting answers all run on the Upform free plan, which is permanent, not a trial. There is no cap on responses, and logic and every integration are included. Pro is $12 a month and removes Upform branding, makes file storage unlimited and adds up to five team members.

What questions should a product feedback survey ask?

Four do most of the work: which feature the person uses most, how well it does the job, what gets in the way, and what is missing that they expected to find. Add a permission question before you ask for an email, so you only follow up with people who agreed. Past seven questions, each extra screen is one more place for someone to stop.

How is this different from an NPS survey?

NPS asks how likely someone is to recommend your whole product, on a 0 to 10 scale, and is usually sent on a fixed cycle. This form asks how well one feature works, on a 1 to 5 rating, and what is missing from it. Use NPS to track the relationship and this template to decide what to fix or build next.

Why does question 3 only appear for some people?

Because asking a happy user what gets in the way wastes their time and yours. A logic rule shows question 3 only when the rating on question 2 is less than 4, so a score of 1, 2 or 3 gets a follow-up and a 4 or 5 moves straight on. A second rule shows the email field only after someone agrees to be contacted.

Can people attach screenshots?

Yes. Question 5 is an optional File Upload, and you choose the allowed file types and the maximum file size in its settings. Uploads count toward storage, which is 100 MB on the free plan and unlimited on Pro. Each file stays attached to its response in the responses table.

Can the feedback go straight into Notion or Slack?

Both. Connect Slack and every response posts to the channel you pick, with the answers you choose to include. Connect Notion and each response becomes a row in a database you have shared with the connection, using your own field mapping. HubSpot, Google Sheets, monday.com, n8n and custom webhooks are included on the free plan too.
